Terms and Conditions

By participating in programs, related events, activities, and using equipment provided by ALLWIN Services Pte Ltd (“ALLWIN”), you (the “Participant”) agree to the following terms and conditions:

Certification of Capability

The Participant certifies that they and/or any minors for whom they are responsible are physically and mentally capable of participating in the activities and using the facilities. The Participant declares that the information provided in this form is true and correct to the best of their knowledge and belief.

Authority to Agree

The Participant certifies that they are agreeing to this document on their own behalf and/or as the parent/legal guardian of the listed participant, with full authority to agree to these terms on their behalf. The Participant acknowledges that this document is a legally binding agreement and confirms that they have had sufficient opportunity to read the entire document.

Compliance with Terms

The Participant agrees to comply with all terms and conditions of participation. If the Participant observes any unusual or significant hazards during their presence or participation, they will immediately remove themselves from participation and notify the nearest official.

Compliance with Data and Facility Rules

The Participant agrees to abide by the rules of the facility and consents to the use of their data in accordance with Singapore’s Personal Data Protection Act.

Consent to Treatment

By attending an appointment at ALLWIN, the participant acknowledges that they willingly give consent to ALLWIN’s ownership and employees to perform any and all necessary examinations and treatment procedures in accordance with their professional training, qualifications, and understanding of the participant’s situation. The participant understands that, during service rendered, different health practitioners at ALLWIN may be involved in their care.

The Participant understands that they have the opportunity to discuss the nature and purpose of their participation before any activity is performed, and that they may opt out of their participation at any time.

Health Statement

The Participant agrees to notify ALLWIN’s ownership or employees if they suffer from any medical or health condition that may cause injury to themselves or others or require emergency care during participation.

Media Consent

By agreeing to these terms, the Participant grants ALLWIN all rights, titles, and interests in any recordings of their name, image, voice, or statements, including any photographic images and video or audio recordings made by ALLWIN.

Data Use Consent

The Participant agrees that ALLWIN may, for the purpose of its Healthy CEO Club, collect, use, and disclose their personal data as provided in the application form, or obtained as a result of membership, for purposes outlined in Singapore’s Personal Data Protection Act, including but not limited to:

  • Processing the membership application;
  • Administering health challenges;
  • Managing the membership with the organisation.

Intellectual Property Rights

All copyrights, trademarks, portrait rights, and other related rights to this product belong to ALLWIN or its partner companies unless otherwise specified. Participants are prohibited from using, transposing, copying, modifying, disclosing, or permitting the disclosure, publication, or communication of any materials obtained from the product for any purpose other than personal use, as specified by copyright laws, without prior consent from ALLWIN or the rights holders.

Variability of Terms

ALLWIN reserves the right to modify, change, or discontinue any part of the product or agreement, including but not limited to pricing structure, club structure, and services provided. ALLWIN also reserves the right to modify or change these terms and conditions at any time without prior notice.

Waiver and Release

The Participant acknowledges that participation is voluntary and chooses to participate despite the risks. To the fullest extent permitted by law, the Participant voluntarily waives, releases, discharges, and agrees to indemnify and hold harmless ALLWIN from any and all liabilities, claims, demands, obligations, or causes of action for any injury, death, property damage, or other loss that may arise from or be connected to the Participant’s involvement in these activities or use of ALLWIN’s equipment or facilities, whether such activities or usage are supervised or unsupervised, or arise from the negligence of ALLWIN or others.

Assumption of Risk

The Participant acknowledges and agrees that participation in the program involves significant risks, both known and unknown, which could result in physical or emotional injury, paralysis, illness, death, or damage to the Participant, their property, or third parties. These risks include, but are not limited to, falling, colliding with equipment or other obstacles, dropped items, equipment failure, failure to follow safety procedures, or negligence by other participants, spectators, facility users, third parties, or ALLWIN.

Acceptance of Risk

The Participant knowingly and voluntarily assumes all risks, both known and unknown, associated with these activities, including those arising from the negligence of ALLWIN or others, which may result in injury, death, property damage, or other loss.

Indemnification

The Participant agrees to indemnify and hold harmless ALLWIN from all liabilities or claims made by other individuals or entities as a result of the Participant’s actions or the actions of their children, parents, heirs, representatives, successors, executors, administrators, and assigns.

Acknowledgement of Terms

The Participant confirms that they have read, understood, and accepted these terms and conditions, acknowledges that they have given up substantial rights by agreeing to them, and agrees to them freely and voluntarily without any inducement.

Direct Payment

When the Participant is required to pay the full cost of the service, payment will be accepted via PayNow or Stripe on the premises at the time of each appointment or beforehand. ALLWIN can also provide a full receipt by email if the Participant needs to reclaim this charge from their insurer.

Late Payment

Any ongoing service may be suspended or canceled if fees and expenses are not paid in full when due. In the unfortunate event that payment enforcement is required, the Participant will be liable for all incurred costs.

Cancellation Policy

Reminder emails and/or text messages may be sent as a courtesy to all clients; however, it is the Participant’s responsibility to keep a record of all future appointments. To cancel or reschedule an appointment without incurring a charge, we require at least 24 hours’ notice. Appointments canceled or rescheduled within 24 hours of the appointment time will be subject to a late cancellation charge, which will be the full fee of the appointment charged at our self-funding rates.

Membership Termination

ALLWIN reserves the right to cancel a Participant’s Healthy CEO Club membership immediately, without prior notice, if any of the following conditions apply:

  • The Participant provided false or inaccurate information when applying for membership.
  • The Participant’s membership has been revoked or they have been censured for violating these terms and conditions or other regulations in the past.
  • The Participant engages in activities prohibited by these terms and conditions or other ALLWIN regulations.
  • The Participant fails to participate in health challenges.
  • The Participant cannot be contacted via email, telephone, or other means.
  • ALLWIN deems any other act by the Participant as inappropriate.

Referral

When further care or imaging is required, we may refer the participant to a third party, for which we will send a referral letter. After this, it is the Participant’s responsibility to arrange an appointment with the third party. Once under the care of the third party, the Participant will be bound by the terms and conditions of that third party, and we assume no liability with respect to that third party.

Transferring Packages

Personal training packages are transferable and remain valid for one year from the date of purchase.

Money Back Guarantee

If the Participant is not completely satisfied with the personal training services rendered, a full refund will be provided, provided that:

The request is made within 30 days of the first session.

The request is made with fewer than three sessions rendered.

There is a valid reason for the refund.

Insurance claims have not been made for services rendered.
